Provider Number - Select your provider type from the dropdown list and enter the number in the textbox.
Employer Name - Enter full or partial Employer name. For example, if you enter the characters 'Sunshine' in the Employer name, all facilities that include the phrase 'sunshine' in the name will be returned.
County - Select a County from the dropdown list to limit search results to Employers in that County.
